If you are a Medicare patient age 65 and over with either 2 or more chronic conditions (i.e., dementia, diabetes, depression, arthritis, osteoporosis, hypertension, etc.) or one longstanding symptom, you qualify for chronic care management.
Annual Wellness Visit
As part of the Affordable Care Act, if you are over 65 Medicare allows you to receive an Annual Wellness Visit once per year.
The Annual Wellness Visit is a comprehensive wellness screening evaluation that creates the care plan your medical provider will use to guide your health and help qualify you to participate in Chronic Care Management.
